APPOINTMENTS

Please call the office to make an appointment in advance. If you are unable to keep your appointment, please call us as far in advance as possible in order for us to reschedule that time slot. We will make a sincere effort to adhere to our appointment schedule and appreciate your patience if we are late due to emergencies or hospital surgery.

OFFICE VISITS

If this is your first evaluation in our office for your current problem, it is important for you to bring along any X-rays, test results, or office records that may be previously been done concerning the present problem.

Our office will assist in scheduling any X-rays, physical therapy, or referral appointments that may be necessary as a part of your treatment.

Please remember that it is our policy that no smoking be allowed in our office or waiting room.

TELEPHONE

The office telephone number is 408-297-3484 and is answered by our staff during office hours. Our fax number is 408- 292-6481. Calls concerning appointments, prescription refills, billing or insurance procedures should be handled during office hours by our staff. Prescriptions will not be called in after office hours. Please allow us several hours to call your prescription in.

If you have a medical question or problem, your call will be directed to a nurse. If necessary, she or your doctor will return your call. Emergency calls will be handled immediately.

FEES & PAYMENTS

We accept cash, checks and most credit cards for your convenience. We will bill your insurance company, however, payment is the patient's responsibility. We will help in ever way we can to assist in handling claims.

INSURANCE

Dr. Anderson is a member of most area Preferred Provider organization and accepts assignment from Medicare. Worker's Compensation insurance cases are also accepted.

PRESCRIPTIONS & RENEWALS

Please request all prescriptions and authorizations for renewals during normal office hours when our full records are available.

PRESENTATIONS

Group talks and slide presentations are available by contacting the office. Subjects include joint replacement, arthritis, and sports medicine. Free literature is available in the office.
